Clinical Outcomes and Complications After Liposuction: A Systematic Review and Meta-Analysis

Background: Liposuction is a common surgical procedure used to remove excess adipose tissue in targeted regions of the body. Despite its increasing popularity, current literature lacks a cohesive understanding of the risks associated with this aesthetic procedure. This study represents the first meta-analysis of its kind to explore the risks associated with liposuction.

Methods: A systematic review and meta-analysis was reported according to PRISMA guidelines and registered on the PROSPERO database. The primary outcome of the study was the overall rate of complications. Absolute risk for individual complications was also assessed. Ultrasound-assisted, power-assisted, laser-assisted, and radio-assisted liposuction were included in this review, while high-definition liposuction was excluded.

Results: Out of 2,957 articles resulting from initial search, 39 studies were included for analysis. In total, 29,368 patients were included, with a mean age of 40.62 years and mean BMI of 26.36. Overall, the absolute risk of any complication was 2.62 [95% CI: 1.78, 3.84]. Additionally, significant risk of developing the following complications was found: contour deformity (2.3 [95% CI: 1.05, 5.16]), seroma (0.65 [95% CI: 0.33, 1.24]), hematoma (0.27 [95% CI: 0.12, 0.60]), infection (0.20 [95% CI: 0.082, 0.50]), allergic reaction (0.16 [95% CI: 0.050, 0.52]), and generalized edema (0.041 [95% CI: 0.0051, 0.32]).

Conclusion: Liposuction is a safe procedure with an overall low complication rate. When they do occur, contour deformity and hyperpigmentation are amongst the most prevalent complications. These findings help define expectations for liposuction procedures, improving patient counseling and contributing to improved trust between physician and patient.
